Who should not take CIALIS?

You should not take Cialis (tadalafil) if you use nitrates or nitrate-based chest pain medicine, if you have had a heart attack or stroke in the last 6 months, if you have unstable angina or uncontrolled high blood pressure, or if you have severe liver or kidney disease. You also need a doctor’s review first if you have retinitis pigmentosa, Peyronie’s disease, sickle cell anaemia or a history of priapism. In short: tadalafil is safe for most healthy adult men, but it is not safe for everyone — and the groups below are the ones who must avoid it or get medical clearance first.

Who should not take Cialis? The short answer

Cialis works by relaxing the smooth muscle in blood vessels so blood can flow into the penis. That same relaxing effect lowers blood pressure elsewhere in the body — which is exactly why certain medical conditions make it dangerous. Absolute contraindications in Malaysia follow the same international labelling used by the NPRA-registered product: any nitrate medicine, riociguat, recent cardiovascular events, and severe hepatic or renal impairment.

Everything else on this page falls into a grey zone. Those conditions do not automatically ban tadalafil, but they change the dose, the monitoring, or both. A quick way to remember it:

• Hard stop: nitrates, riociguat, recent heart attack or stroke, severe liver/kidney disease.
• Doctor first: uncontrolled blood pressure, heart failure, arrhythmia, Peyronie’s disease, sickle cell anaemia, bleeding disorders.
• Manage with care: well-controlled hypertension, mild kidney impairment, diabetes, prostate medicines such as alpha-blockers.

Absolute contraindications: never take Cialis with these

If any of these apply to you, tadalafil is off the table completely — not “take a smaller dose”, not “try it once”.

1. Nitrate medicines for chest pain

Nitroglycerin, isosorbide mononitrate, isosorbide dinitrate and similar nitrates combine with tadalafil to cause a dramatic drop in blood pressure. The interaction can be life-threatening. Because Cialis stays in the body far longer than Viagra (36 hours versus about 4–6), the risk window is much wider. Malaysian guidelines are strict: do not take tadalafil within 48 hours of any nitrate. If you carry GTN tablets or a GTN spray for angina, tadalafil is not an option for you.

2. Riociguat and guanylate cyclase stimulators

Riociguat (used for pulmonary arterial hypertension) acts on the same pathway as tadalafil; combining them is contraindicated and can cause symptomatic hypotension.

3. Recent heart attack, stroke or unstable heart disease

Sexual activity itself puts strain on the heart. Tadalafil is contraindicated after a myocardial infarction or stroke within 6 months, and with unstable angina, life-threatening arrhythmia, or NYHA class 2 or worse heart failure.

4. Severe liver or kidney impairment

Tadalafil is cleared mainly by the liver (CYP3A4) and excreted partly through the kidneys. In severe hepatic impairment or severe renal impairment (creatinine clearance below 30 mL/min), blood levels rise unpredictably. Dialysis patients should not use Cialis at all.

Conditions that require a doctor’s review first

These are not absolute bans, but self-prescribing is a bad idea if any of them apply. Each one changes either the dose or the monitoring you need — and mild to moderate kidney impairment, for example, usually still allows a reduced daily dose under supervision.

Uncontrolled high blood pressure or low blood pressure

Tadalafil lowers blood pressure slightly — usually harmless, occasionally not. If your resting blood pressure is consistently above 170/100 mmHg, or below 90/50 mmHg, get it controlled before starting. Men already on two or more antihypertensives should have their blood pressure checked after the first dose.

Heart conditions short of a recent attack

Stable angina, controlled heart failure or a history of arrhythmia do not automatically rule out Cialis, but a cardiologist should confirm you can tolerate the small drop in blood pressure and the exertion of sex. Peyronie’s disease or a bent erection

Peyronie’s disease causes fibrous scar tissue inside the penis and a curvature during erection. Tadalafil does not cause Peyronie’s, but it can make erections firmer and therefore more painful in men who already have it. Get assessed first.

Blood disorders that raise priapism risk

Sickle cell anaemia, leukaemia, multiple myeloma and other clotting or bleeding disorders raise the risk of priapism — a painful erection lasting more than four hours. That is a medical emergency and can cause permanent damage. If you have any of these conditions, only use tadalafil under specialist supervision.

Active stomach ulcer or bleeding disorder

Men with an active peptic ulcer, or who take anticoagulants, should discuss tadalafil with their doctor. There is a small increase in bleeding tendency with PDE5 inhibitors.

Retinitis pigmentosa and vision conditions

Rare cases of non-arteritic anterior ischaemic optic neuropathy (NAION) have been reported with PDE5 inhibitors. Men with retinitis pigmentosa, or who have already lost vision in one eye from NAION, should avoid tadalafil.

Is Cialis safe if you take other medicines?

Drug interactions are the most common real-world reason a man ends up in trouble with tadalafil — far more common than the underlying conditions themselves. The three that matter most in Malaysia:

1. Nitrates (any form) — absolute contraindication, as above.
2. Alpha-blockers for prostate or blood pressure — tamsulosin, doxazosin, terazosin and alfuzosin can cause symptomatic hypotension when combined with tadalafil. The usual fix is a 4-hour separation for tamsulosin and a lower starting dose for the others.
3. Strong CYP3A4 inhibitors — ketoconazole, itraconazole, clarithromycin, ritonavir. These raise tadalafil blood levels sharply; the dose must be capped, and daily Cialis is usually avoided entirely.

What happens if you take Cialis anyway?

Depends which contraindication you ignored. With nitrates, the risk is a sudden, severe drop in blood pressure — dizziness, fainting, in rare cases a cardiac event. With severe kidney or liver disease, tadalafil accumulates and side effects become much more likely. With priapism-risk conditions, the danger is an erection that does not subside.

Seek emergency care immediately if you experience chest pain, fainting, an erection lasting more than 4 hours, or sudden loss of vision.

• List every medicine you take, including supplements and traditional remedies — nitrates hide in heart medications under several brand names.
• Know your recent blood pressure; a home reading from the past month is enough for most men.
• Tell your doctor about any chest pain, breathlessness on exertion, or fainting, even if it was months ago.
• Mention blood disorders, eye conditions and any curvature — these are easy to overlook but change the recommendation.
• Buy only from a legitimate source; counterfeit tadalafil purchased online has been found to contain unknown doses and, in some seizures in Malaysia, undeclared compounds.
